Admissions Process These are the steps and forms you need to complete to matriculate your student at Westlake:

- Parent and Student Interview with the President or Principal.
- Application for Admission must be filled out and signed by a parent or guardian.
- Evaluation / Placement Test (Testing fee is $100.00 nonrefundable and must be made at time of the evaluation).
- At the registration appointment the following forms and procedures will be started and or completed at the appointment:

  • Registration fee and Tuition Deposit is Paid.
  • Previous School Information / Release to Obtain Records / Contact Form Entering students must make arrangements for the transfer of previous grades and records by completing this form which Westlake will forward to your last school.
  • Emergency Procedure / Contact Insurance Waiver must be completed for a parent or guardian.
  • Emergency Contact Card must be completed for a parent or guardian.
  • Dismissal / Pick Up Procedure Form must be completed and returned.
  • High School Driver's Permission Form allowing or limiting a student to leave campus for lunch.
  • Request for Medication Administration If applicable, this form must be completed to administer any form of medication during the school day.
  • Photographs, Sound Recordings and Educational Data Consent Form
  • Physical Education and Activities Permission Form allowing and/or limiting such activities.
  • School Orientation Information must be read, accepted, returned signed.

Please bring to the Registration Appointment the following items:

  • Two (2) recent full face photographs for the school files.
  • Student Physical Examination Form (Yellow) completed by physician. (Must include T.B. Test).
  • Certificate of Immunization (Blue) (Available at physician's office).
  • Copy of Student's Birth Certificate for school records.
  • Copy of Student's Social Security Card or provide the number must be given for school computer records.
  • Copy of Student's Medical Insurance Identification Card

    2. Review the Itemization of Costs Worksheet
    3. Select a Payment Option
    4. Meeting with Admissions Director
    5. Sign the Enrollment Contract

After School Program
School ends at 2:30 pm (Elementary ends at 2:00 pm) and all students must be picked up not later than 3:00 pm.

EXCEPTIONS:

    1. Tutoring from Faculty
    2. Sport Activities
    3. After-School Detention

All students must be picked up right after any of these activities.
Any student staying after hours must be a part of the after-care program, and will be billed accordingly.
