Your decision to consider enrollment at Westlake for your student is one that will benefit your child now and for years to come, as the education and personal skills learned during their time at Westlake become the ‘seeds’ that will allow them to create personal success in their adult life.

Below you will find the necessary forms to enroll at Westlake, in any of the schools three divisions - College Preparatory, Summit Point or Academy.

Please print them out, complete them and bring them with you to your appointment with our admissions office.
